Age Declaration & Consent Form

This section is to be completed by the parent/guardian of the young person wishing to participate in Paintball activities at Revolution Paintball. By law, we are not permitted to allow entry on to the field by anyone under the age of 16. Young persons under 18 (aged 16 or 17) MUST provide this consent form prior to their scheduled booking in order to participate. Revolution Paintball deserves the right to refuse entry to any young person that does not provide this form, completed by a parent/guardian, in accordance with State Law.

Give permission for my son/daughter to take part/play paintball at Revolution Paintball. I understand that, if they do not present this form for acceptance prior to their booking date they will be refused entry in accordance with State Law.

Playing Rules & Regulations

Please read the rules and regulations carefully.

i. All instructions and decisions of referees and Revolution Paintball staff must be followed at all times.
ii. Revolution Paintball will not accept liability whatsoever in respect to a personal injury.
iii. Only approved safety equipment shall be used, all private safety gear must be submitted to Revolution Paintball management for examination prior to use.
iv. Protective clothing including facemask and goggles must remain on at all times unless instructed otherwise by Revolution Paintball staff.
v. Players must ensure barrel blocking devices as provided are fitted at all times whilst not actively participating in paintball games.
vi. Players must observe all safety signs and obey.
vii. No physical contact is to be made during the game.
viii. No player shall fire upon another player at a range closer than five (5) meters.
ix. No player shall deliberately shoot at anyone in the head or face areas not covered by a mask.
x. No player shall deliberately shoot whilst moving between gaming zones.
xi. No smoking is permitted on the playing fields only designated areas near office.
xii. ALL players and visitors must not be under the influence of drugs or alcohol.
xiii. Theft will not be tolerated; you will be prosecuted.
xiv. Any player who is in breach of the game rules will be removed from the playing area. Players who have been evited from the field will forfeit any outstanding paint. No refunds.
xv. Revolution Paintball reserves the right to remove immediately from the game and field any person or group who in which the opinion Revolution Paintball or its staff has breached any of these rules or is engaging in any behaviour which Revolution Paintball or its staff consider be unacceptable or dangerous.
xvi. No agent, servant or representative of the business has the authority to alter, modify or waive any of the above conditions.
xvii. Any brakeage or loss of Revolution Paintball equipment loaned to during the day will be charged to me at cost price.
xviii. When supplying your e-mail address you may receive promotional material.

By signing below, you acknowledge and accept the following:

1. I have read and understood the terms and conditions and the rules and regulations of play. I am fully aware of all the risks involved including the possibility of injury to my son/daughter. I nevertheless accept and assume such risks on his/her behalf. I also agree to be responsible for the cost of any breakages or loss of equipment, and any other monies owed.
2. I herby state that I am making a free and fully informed decision and give my consent for him/her to participate in paintball games at Revolution Paintball.
3. I herby indemnify and shall keep indemnified Revolution Paintball, its officers, employees, agents and contractors against injury, loss or damage suffered by my child or for which he/she might otherwise be responsible.
